
While the goal of periodontal procedures are to eliminate pocketing and maintain bone support, some periodontal procedures are indicated to improve aesthetics or to allow for the maintenance of a severely broken down tooth. One such procedure is periodontal crown lengthening.
This procedure is indicated in the case of a “gummy” smile where the teeth have adequate length but are covered by too much gum tissue. During this procedure, excess gum and bone tissue is reshaped to expose more of the natural tooth. This can be done to one tooth, to even your gum line, or to several teeth to expose a natural smile.
Your dentist may also recommend crown lengthening for restorative reasons. If a tooth is too short to restore or is broken down below the gum line, we can perform a crown lengthening procedure to expose more tooth structure to allow your dentist to save the tooth and to place a long lasting restoration.
Crown lengthening is a periodontal surgical procedure that reshapes gum and sometimes bone tissue to expose more of the natural tooth. It’s used to enhance smile aesthetics (reduce a gummy smile) and to allow proper placement of dental restorations like crowns or fillings.
A dentist may recommend crown lengthening if there isn’t enough visible tooth structure for a secure crown, the tooth is broken below the gumline, or excessive gum tissue hides too much of the tooth. Clinical exams and X-rays help determine candidacy.
During the procedure, local anesthesia is provided, excess gum tissue is removed, and if needed, a small amount of bone is contoured to expose more tooth structure. The gum is then sutured to allow for proper healing.
Most patients feel little to no pain during the procedure due to local anesthesia. Mild discomfort or sensitivity afterward is normal and can be managed with prescribed or over-the-counter pain relief.
Initial healing usually takes about 1–2 weeks. Full healing and stabilization of gum tissue can take several weeks to a few months before final restorative work, such as crown placement, can be completed.
Yes. Crown lengthening can improve the appearance of a gummy smile by revealing more of the tooth surface, resulting in a more balanced and aesthetically pleasing smile.
Potential risks include temporary sensitivity to hot and cold, mild bleeding, swelling, or infection if postoperative care instructions are not followed. In some cases, teeth may appear slightly longer after healing.
Preparation may include a dental exam, X-rays, and a discussion of your medical history and medications. A professional cleaning or temporary restorations may be recommended before surgery.

Coverage varies by provider and policy. If the procedure is deemed medically necessary, such as to place a crown or restore a tooth, insurance may cover part of the cost.
